There are always options:
1) Call the office and talk with Dr. Shaar or Angela on how to take your first step.

2) Look on the website directory for forms. Download the form for the patient you would like to or need to be. Please fill this patient intake form out as completely possible and bring it with you. You will want to gather any x-ray and/or MRI films or reports to have them with your too at your first appointment.
3) Insurance situations are pretty straight forward.
- There are typically 3 different types of insurance situations:
- We are only contracted as an in-network provider with Humana.
- If you do not have Humana insurance we can typically work with other insurance companies that have a PPO policy. Typically these types of insurance policies have a deductible that the patient must meet first before the insurance benefits will start paying.
- We can not work with HMO policies, unless they are Humana.
